Case Management

About the Program

Case Management is designed to assist clients desiring to become self-sufficient. This includes reviewing a client’s overall needs and developing a plan of action to meet those needs. Clients with a need greater than Level 1 or 2 and who demonstrates the desire to set and meet goals, will be invited to participate in the case management program. This begins with staff conducting an initial orientation advising the client of the benefits of participating in case management and a description of the process and requirements of the program. Potential case management clients are informed of available workshops. Clients also receive a brochure with information on the case management program. Clients who do not wish to participate in case management have their immediate needs met. The case management program is reintroduced to them at a later date.

In addition to case management, the Community Services Block Grant provides emergency services, as well as information and referrals. CCSCT has a long-standing relationship with area providers and maintains a book of resources in an effort to provide clients with assistance as needs arise.

Information and Referral Services are offered to help clients with their individual needs. Coordination with local service providers is essential in assuring that clients receive necessary services. Each CCSCT Community Service office maintains a directory of local service providers within their county.

Emergency Crisis Intervention Services are offered to clients requiring immediate assistance (i.e. housing utilities, food, clothing, transportation, etc.)

Service Area

The counties serviced by the Community Services Block Grant (CSBG) include: Atascosa, Bandera, Comal, Edwards, Frio, Gillespie, Guadalupe, Karnes, Kendall, Kerr, Kinney, Live Oak, McMullen, Medina, Real, Uvalde, Val Verde, Wilson, and Zavala.
